Enrollment Application

If you're interested in enrolling, please complete the following:

• Application

• Enrollment packet

• Copy of birth certificate

• Copy of up-to-date immunizations

• Copy of up-to-date physical

• Food transportation form (if applicable) 

• Parent Notice Form

• Additional documents for infants/enrolling more than one child. 

• J's Clubhouse 2025 Closing List​
